Use one completion row per material
List the room, wall or floor reference and material exactly as used during monitoring. If the provider did not monitor a material or could not access it, record that limitation rather than assigning a completion status.

Ask for the final date, location, method, reported result, comparison basis and provider conclusion. Link it to the earlier sequence so the decision is not represented by an isolated number.

Keep limitations and exclusions open
Record areas that remain inaccessible, require specialist review, await source repair or are excluded from the agreement. A completion statement for one material should not silently close another material or room.
For each equipment pickup, record the related room decision. Then separately list open trim, paint, flooring, cabinet or reconstruction work and the provider receiving the handoff.
